Parental Consent and Understanding

I confirm that I have read the Club’s Code of Conduct and that I and my child will adhere to it

I confirm that I have read or been made aware of the Club’s policies concerning safeguarding Children, Anti-Bullying, Online Safety and Social Media, and I understand and agree to the responsibilities which I have regarding these policies

I consent to the Club photographing or videoing my child’s involvement in cricket in line with the ECB Photography Filming and Social Media Broadcasting guidelines

I consent to the Club publishing photographs or articles on the Club website which may include or reference my child

I confirm that if my child is playing in an Adult team, they may change in the same changing room as the Adult players

I confirm that my child will comply with the rule that all Club Members under the age of 18 playing any form of hardball cricket (incl. Net practice) will wear a helmet when batting or keeping wicket in a match or training

I give consent that in the case of injury, accident, or illness whilst in the care of Middleton & North Dalton Cricket Club that a Club Officer or Adult Member of the Club may give permission on my behalf for the carrying out of ANY medical treatment, including blood transfusion, and surgery on my child as considered necessary by a qualified medical practitioner

I hereby agree to indemnify Middelton & North Dalton Cricket Club, its officers, coaches, managers and volunteers, from any liability for loss or damage to personal property, or accident or injury to my Child which occurs whilst they are receiving coaching, playing cricket matches or participating in any club event or activity.

Data Privacy Notice

Middleton & North Dalton Cricket Club take the privacy of our members data seriously and will only use your and your child’s personal information for club administration purposes and for communicating with you about your child’s membership and involvement in Club activities.

For example:

- To administrate your membership

- For Player Registration to Cricket Leagues (where applicable)

- For Player availability and match selection (where applicable)

- To inform you of club events & activities

(e.g. Presentation Evenings, Net Sessions (if applicable) etc).

Only nominated committee members/adult members of the club namely the Chairman, Secretary, Treasurer, Coaches (where appropriate), Senior Team Captains (where appropriate) and the Club’s League Administrators (where appropriate) will be able to access your details.

Note: If your Child plays for one of the adult league teams we will also require their Date of Birth, Nationality and years of residence in the UK if a non-UK passport in order to register them with the appropriate league in order for them to be allowed to play.

Your data will not be shared with any third party and the principles of the General Data Protection Regulation will be adhered to.
